Background Check for FL HAZMAT CDL

To apply for or renew a HAZMAT endorsement with your commercial driver’s license, you’ll need to be pass a Transportation Security Administration (TSA) background check. If you’re renewing, start the process at least 30 daysbefore your license expires*.

In Florida you request a background check at a DHSMV licensing office. When you visit the office, provide your:

• CompletedDriver's License Hazardous Materials Endorsement Application(Form HSMV 71141).
  • You can enter the information online and print the completed form by using the DSHSMV web portal.
• Payment for the $91 background check fee.

Once you’ve finished the application process and:
